REGISTRATION  DOCUMENTS

These documents are REQUIRED for students enrolling in classes funded by the district.

OVA School of Choice for the Fall 2024-2025 school year will be from
March 1, 2024 to August 14, 2024 for grades DK - 12.

VIRTUAL CONSENT

Print 1 copy (up to 3 children).

If registering more than 3 students, please print a 2nd copy.

    HEALTH  APPRAISAL  FORM

    Kindergarten  Form -

      1st - 12th Grade  Form -

      SCHOOL OF CHOICE (SOC)

      Print 1 per student (2 page form)

      *If a student has an IEP or 504, a copy MUST also be provided with this document.
       

      Parental Notice - This form is only for families that do not reside in Oakland County.

        TRANSCRIPT / REPORT CARD 
        REQUEST

        Print 1 per student.
         

        • If your student was homeschooled, you are still required to submit this form. For previous school, list "Homeschooled".

         

        • Kindergarten students are still required to submit this form. For last grade completed, list "None"
